We offer Pediatric Sick and Well Visits, Sports Physicals, and Acute Minor Sick Visits for Adult clients who are immediate family members of the Pediatric patients that we care for. And yes this is all done from the comfort of your own home. Aside from being convenient in more ways than one, we hope that these services will eliminate stress, anxiety, and time constraints, that may be associated with a clinic visit for some.

Meet the provider
Monica Closner-Kozlowski
Monica Closner–Kozlowski is a Board Certified Family Nurse Practitioner with 15 years experience in Pediatric Care. She has provided care to patients in inpatient, and outpatient settings. This has included a Texas Children’s hospital, and local Albuquerque urgent care and primary care settings. She is a member of the American Academy of Nurse Practitioners as well as the American Academy of Pediatrics.

Well Child Checks
Developmental screenings, physical exam, anticipatory guidance, vaccination, diagnosis, and treatment.
Sick Visits
We provide diagnosis and treatment for minor sick illness to include sore throat, upper respiratory illness, Influenza, UTI, asthma management, vomiting and diarrhea, rashes, minor sprains, seasonal allergies, sinus infections, ear infections, eye infections.
Sports Physicals
Get your child ready for this year of athletics by scheduling your child’s sport physical quickly and conveniently
Adult Care
As an added feature to our Pediatric services, she provides adult care to immediate family members of the Pediatric patient/s that she is visiting
Preventative Care is Key
Preventative care is key in promoting a safe, healthy, and happy life. Well Child Checks are offered for Pediatric patients ages newborn to 18 years of age. These include developmental screenings, physical exam, anticipatory guidance, vaccination, diagnosis, and treatment.

Payment will be collected at the time of the visit. Additional fees will apply for medications administered or screenings/tests performed during the visit. We accept most credit cards including American Express
We Accept Insurance:
We accept insurance for Presbyterian, United Healthcare, Blue Cross Blue Shield, Medicaid, Cigna and Aetna. Please be familiar with your insurance plan and coverage for Sick and Well Child Visits.
Co pay is charged as a standard clinic / PCP visit so please be familiar with what your coverage entails in terms of standard clinic/PCP clinic visits.
